Myxofibrosarcoma is a rare type of highly malignant sarcoma which easily occur lung metastasis. We report metastatic myxofibrosarcoma generated from the upper and middle lobe lung, invading right superior pulmonary vein, extending to left atrium. A single imaging examination may lead misdiagnosis, so combination computed tomography imaging, magnetic resonance as well positron emission tomographic scan are necessary for perioperative diagnosis surgical planning.
