Background Cardiogenic shock carries high mortality. This study investigated the relationship between protocol‐advocated best practices and outcomes. Methods Patients with cardiogenic supported by Impella CP in an Asian tertiary cardiac center were evaluated for 30‐day post percutaneous ventricular assist device (PVAD) survival after adopting a standardized protocol emphasizing early mechanical circulatory support (shock‐to‐PVAD time ≤180 minutes), pulmonary artery catheterization invasive...

A 70-year-old man with history of transcatheter aortic valve-in-valve implantation was admitted because suspected infective endocarditis (IE). Transesophageal echocardiogram did not reveal any vegetations, as the metallic stent frames caused significant artifacts. Position emission tomography also negative. Intracardiac (ICE) performed retrogradely through ascending aorta, which showed clear vegetations over frame heart valve. Endocarditis after valve uncommon. With increasing use...

Transcatheter aortic valve replacement (TAVR) is often used to treat severe stenosis in younger patients with a longer life expectancy. These may out live their index transcatheter (TAV) and Heart Teams must anticipate plan for future redo-TAVR. This includes the choice of TAV its deployment position which potentially preclude redo-TAVR coronary access. New models assess mitigate risk obstruction or inaccessibility TAV-in-TAV are needed prior performance procedure (see Fig. 1). As...

Traditionally, percutaneous mitral commissurotomy (PMC) is performed under fluoroscopy only. In difficult cases, general anesthesia with transesophageal echocardiography (TEE) guidance needed. Intracardiac (ICE) enables operators to perform PMC local while providing intra-procedural imaging guidance, as in TEE. Hereby, we describe a case of guided by ICE allow early detection complications.
