鼻咽癌很少以昏厥作為一開始的臨床表現,此篇報告1名55歲女性病患,主訴為失去意識一段時間被送至本院急診,到院時並無發現明顯的神經理學檢查異常,也沒有明顯的顱内病灶,然而在頭頸影像學檢查發現右側淋巴結腫大及右側鼻咽部顯影。右側鼻咽部切片,病理報告為未分化未角化癌,癌症分期結果為鼻咽癌,cT2N2M0,第三期。右側頸部淋巴結病變包圍著頸動脈分叉,判斷為腫大淋巴結壓迫到頸動脈竇引起昏厥。我們使用升壓口服藥物Midodrine來預防昏厥發生,後續接受併用化學放射療法,經過一段時間治療,昏厥在沒有使用藥物的情況下也已改善。臨床上很難從暈厥表現發現鼻咽癌,若能早期發現,早期介入治療,就能改善病患預後。
