We present a case of 63-year-old male with an 8-year history left-sided cystic facial mass which recurred despite multiple drainage procedures. Imaging findings showed in the left parotid gland and it was surgically resected. Pathology confirmed to represent mammary analogue secretory carcinoma, relatively newly described entity. This illustrates that carcinoma can masquerade as cyst within gland.
