Point-of-care ultrasound (PoCUS) has been integrated into undergraduate medical education. The COVID-19 pandemic forced schools to evolve clinical rotations minimize interruption through implementation of novel remote learning courses. To address the students' need for education, we created a virtual PoCUS course our fourth-year class. We present a case of 63-year-old male with an 8-year history left-sided cystic facial mass which recurred despite multiple drainage procedures. Imaging findings showed in the left parotid gland and it was surgically resected. Pathology confirmed to represent mammary analogue secretory carcinoma, relatively newly described entity. This illustrates that carcinoma can masquerade as cyst within gland.
