嚢胞腎を合併し,正常レニン活性値を示した原発性アルドステロン症の1例を経験した.本症例は39才,女性.主訴は高血圧,脱力感,低カリウム血症.副腎シンチグラム,副腎静脈造影,副腎静脈採血などにより,右副腎腺腫による原発性アルドステロン症と診断され,右副腎摘出術が施行された.嚢胞腎を合併していたが,腎機能障害は軽度であつた.血漿アルドステロン値が高値を示したにもかかわらず血漿レニン活性は正常範囲であつた.手術後降圧を認め,血清カリウム値も正常化したが,血漿レニン活性は高値を示した.嚢胞腎は腎不全が進展しないうちより,高頻度に高血圧を示すが,その原因の一部にレニンーアンジオテンシン系の関与するものがあると言われている.本症例の術前における血漿レニン活性が抑制されなかつた原因としては嚢胞腎の合併によるものと考えられた.

反芻と周産期疾患との関連性について比較解析を行った.発情検知用として市販されている,反芻時間を自動計測するセンサー(反芻センサー)を周産期の乳牛の頸部に装着して反芻時間を計測するとともに,臨床観察および血液生化学検査を実施して反芻時間との関連性について比較した.その結果,周産期疾患を発症した6頭の反芻時間は,分娩14日前から分娩日にかけて周産期疾患を発症していない16頭と比較して低く推移した.また,分娩14日前から1日前までの反芻時間の平均値と分娩日の血清中NEFAの間に負の相関が認められた(r=-0.51,P < 0.05).更に,分娩日の反芻時間と分娩14日後の血清中BHBAの間に負の相関が認められた(r=-0.44,P 0.05).以上から,分娩前の反芻時間が短い場合牛は周産期疾患になりやすく,分娩前および分娩日の反芻時間とケトーシス発症の間に関連性がある可能性が示唆された.
