要旨:急性内頸動脈閉塞症のうち,内頸動脈終末部以遠が側副血行で灌流される急性内頸動脈非終末部閉塞症(non-T ICA occlusion)に関して,慢性期に自然再開通を確認した自験例5 例について報告する.発症時は5 例とも軽症であり,閉塞側に血栓溶解療法や急性期血行再建術は行わず,内科治療のみを選択した.再開通は閉塞後39~859 日(中央値69 日)に確認した.2 例で再開通後に動脈解離を疑う所見を認めた.4 例は閉塞から自然再開通まで再発なく経過したが,1 例で閉塞側の側脳室後角周囲に無症候性脳梗塞を生じた.再開通後に内頸動脈起始部に高度狭窄が残存した2 例に対しCEA を施行した.急性内頸動脈閉塞症は慢性期に自然再開通することがあり,軽症の急性non-T occlusion であった場合でも,再開通に関連して虚血性脳卒中の再発を来したり高度狭窄が残存する例が存在し得るため,慎重な経過観察が望ましいと考えられた.
要旨:両側主幹動脈塞栓症は全虚血性脳卒中の0.34%に生じたとの報告があり,治療例に関する報告は少数で予後も不良である.両側内頸動脈系塞栓症に対し機械的血栓回収療法が奏功した症例を報告する.心房細動未治療の69歳男性,意識障害,右共同偏視,左上下肢麻痺,失語で救急搬送となった.来院時,JCS 20,NIHSS 28点.ASPECTS-DWIは右が7点,左が10点であった.MRAでは後交通動脈分岐遠位の右内頸動脈,左中大脳動脈閉塞を認めた.rt-PA静注し両側一期的血栓回収術を施行.ASPECTSがより低値で症状が重い右側から治療した.右側mTICI 3,左側2aの再開通を得た.術翌日MRIで脳梗塞の進行を認めずNIHSSは1点に改善,mRS 0で自宅退院した.両側内頸動脈系塞栓症は保存的加療の予後が悪いが,症例によって治療順序(左右)の選択を適切に行い,迅速に血管内治療を遂行すれば良好な予後が見込める可能性がある.
