This paper introduces ClimateGPT, a model family of domain-specific large language models that synthesize interdisciplinary research on climate change. We trained two 7B from scratch science-oriented dataset 300B tokens. For the first model, 4.2B tokens were included during pre-training and second was adapted to domain after pre-training. Additionally, ClimateGPT-7B, 13B 70B are continuously pre-trained Llama~2 Each is instruction fine-tuned high-quality human-generated has been created in...
» Arthrofibrosis after total knee arthroplasty (TKA) is the new formation of excessive scar tissue that results in limited ROM, pain, and functional deficits.» The diagnosis arthrofibrosis based on patient's history, clinical examination, absence alternative diagnoses from diagnostic testing, operative findings. Imaging helpful ruling out specific causes stiffness TKA. A biopsy not indicated, no biomarkers exist.» pathophysiology multifactorial related to aberrant activation proliferation...
Preparation of a single cell suspension from solid tissue is vital for successful flow cytometry experiment. We report detailed and reproducible method to produce quality the zebrafish retina. Zebrafish retinas, especially their Müller glia cells, are particular interest inherent regenerative capacity, making them useful model medicine therapy research. Here, we detail papain-based dissociation that gentle enough keep cells intact, but strong disrupt cell-cell cell-matrix interactions yield...
Abstract Hospital readmissions after primary total knee arthroplasty (TKA) significantly drive health care expenditure and resource utilization. Recent studies have suggested differences between medical orthopaedic TKA their episodes of (EOCs) but lack patient-level data reporting. This study aimed to compare EOCs for orthopaedic-related regarding initial readmission wards, services consulted, intensive unit (ICU) admissions, blood transfusions, surgical interventions, length stay, discharge...
To describe a case of primary vitreoretinal lymphoma with central nervous system involvement in patient sarcoidosis.
Purpose: To investigate whether any systemic medical conditions may be associated with a higher risk for developing postinjection endophthalmitis. Methods: This case-control study is retrospective review within the Emory Eye Center from 2009 to 2019 and The Cleveland Clinic Foundation 2012 2019. Each case was matched in 1:4 case-to-control ratio. associations between comorbidities endophthalmitis were explored using multivariable logistic regression models on combined sample. Results:...
In this case series, 3 unique cases of severe immune-mediated pathologies (primary ocular lymphoma, toxoplasmosis chorioretinitis, and syphilitic retinitis) after dropless cataract surgery with intraocular steroid injection are reported. Patient immune status subsequent local immunosuppression played roles in the worsening these pathologies. This report demonstrates need for vigilance when administering immunosuppressive agents locally during surgery.
Background To report the case of a patient with two distinct genetic systemic diseases – pseudoxanthoma elasticum (PXE) and Usher syndrome confirmed by testing.Materials Methods Single Retrospective Case ReportResults A 36-year-old woman presented acute central vision loss left eye (OS). Fundus exam revealed choroidal neovascularization OS in setting angioid streaks secondary to an underlying diagnosis PXE. Unexpectedly, she also exhibited peripheral bony spicules significant visual field...
An 84-year-old woman presented 6 years after revision total hip arthroplasty (rTHA) with worsening pain and a Paprosky classification IIIB femoral defect. rTHA was performed using proximal femur replacement. Given her osteoporosis poor bone stock, tibial cone impaction grafting (IG) were used for megaprosthesis fixation. At the 33-month follow-up, patient pain-free radiographs demonstrated well-fixed implant. In setting of massive defects poor-quality bone, novel use IG can be implemented to...
Subarticular cystic lesions, also known as geodes, present a challenge in the management of patients undergoing primary total knee arthroplasty (TKA). Although multiple treatment options are available for addressing these uncertainty persists regarding optimal approach.A 58-year-old man with history rheumatoid arthritis presented several years left pain. Evaluation showed severe degenerative osteoarthritis complicated by presence large lateral femoral condyle cyst. After failing conservative...
Introduction: Indications for patellofemoral arthroplasty (PFA) remain under debate, with many surgeons favoring total knee (TKA) even in patients limited degeneration the tibiofemoral compartments. Case Report: A 30-year-old man a history of bilateral patellar instability treated youth multiple surgeries presented 8+ months recurrent pain. Evaluation showed severe arthritis preserved medial and lateral Bilateral sequential robotic-assisted PFAs (RA-PFA) were offered as precise approach to...
