頸部リンパ節は舌癌の予後に影響する。1986年から2004年までにおけるstage I,II舌癌113例の検討を行った。26例に予防的頸部郭清術が行われ,これら8例に一次転移が認められた。一方,87例には予防的頸部郭清術は行わず,26例に後発転移が認められた。今回の検討において,悪性度とリンパ節転移に明らかな関連性は示されなかった。一方,浸潤様式はリンパ節転移に有意な因子であった。5年累積生存率に関しては一次転移群が87.5%,後発転移群が57.5%であった。頸部死,遠隔転移死はそれぞれ6例に認められた。また,被膜外浸潤は,後発転移群のみに認められた。これらの結果より,リンパ節転移において浸潤様式と被膜外浸潤が重要な予後因子であることが示唆された。Stage I,II舌癌における予防的郭清術の適用について,さらなる研究を検討するべきであろう。
