Veno-venous extracorporeal membrane oxygenation (V-V ECMO) support is increasingly used in the management of COVID-19-related acute respiratory distress syndrome (ARDS). However, clinical decision-making to initiate V-V ECMO for severe COVID-19 still remains unclear. In order determine optimal timing and patient selection, we investigated outcomes both non-COVID-19 patients undergoing support.
Blood stream infection (BSI) is a potentially lethal complication in patients receiving extracorporeal membrane oxygenation (ECMO). It may be particularly common with veno-venous ECMO due to their long hospitalization the intensive care unit. Given that these have concurrent indwelling central venous catheters (CVC), it unclear whether circuit, CVC, or both, contribute BSI. This study evaluated risk factors associated BSI single institution of 61 from 2016 through 2019. All and circuit...
Veno-venous extracorporeal membrane oxygenation (VV-ECMO) has emerged as a potential life-saving treatment for patients with acute respiratory failure. Given the accumulating literature supporting use of VV-ECMO without therapeutic levels anticoagulation, it might be feasible to planned intubation before surgical procedures. Here, we report consecutive series who underwent initiation VV-ECMO, induction general anesthesia anticipated difficult airways or decompensation. We describe approach...
Background Shorter length of stay (LOS) is a welcome consequence optimized perioperative care. However, accelerated hospital discharge may have unintended consequences. Before implementing an institutional enhanced recovery after surgery protocol, we evaluated the safety shorter LOS and compared outcomes patients with (LOS ≤ 3 days) to those longer > days). Methods We identified all undergoing elective cardiac cardiopulmonary bypass between July 2004 June 2017. Transcatheter approaches,...

Background Limb ischemia is a major complication of femoral venoarterial extracorporeal membrane oxygenation (VA-ECMO). Use ankle-brachial index (ABI) to monitor limb perfusion in VA-ECMO has not been described. We report our experience monitoring patients with serial ABI and the relationships between near infrared spectroscopy (NIRS). Methods This retrospective single-center review consecutive adult placed on January 2019 October 2019. Data were collected paired NIRS values. Relationships...

Background
Fibromuscular dysplasia (FMD) is the most frequent cause of renovascular hypertension in women fertile age. We present a 36 year old identified early pregnancy with severe resistant caused by bilateral FMD.
Case
A 36-year-old primigravida no past medical history was referred to renal-obstetric services at 11 weeks gestation hypertension. Mean ambulatory blood pressure (BP) 180/100 despite maximum treatment dose nifedipine and labetalol. Renal function, ECG,...
