Eleven cases of community-acquired Pneumocystis carinii pneumonia occurred between 1979 and 1981 prompted clinical immunologic evaluation the patients. Young men who were drug abusers (seven patients), homosexuals (six), or both (two) presented with pneumonia. Immunologic testing revealed that absolute lymphocyte counts, T-cell proliferation depressed, humoral immunity was intact. Of 11 patients, one found to have Kaposi's sarcoma, another had angioimmunoblastic lymphadenopathy. Eight...
Pyogenic liver abscess, a potentially life-threatening disease, has undergone significant changes in epidemiology, management, and mortality over the past several decades. We reviewed data for patients admitted to Bellevue Hospital New York University Downtown (New York, York) 10-year period.Of 79 cases reviewed, 43% occurred with underlying biliary disease. The most common symptoms were fever, chills, right upper quadrant pain or tenderness. laboratory abnormalities an elevated white blood...
Pneumocystis carinii pneumonia (PCP) continues to be the most common index diagnosis in acquired immunodeficiency syndrome (AIDS), but it is not clear which of several available agents effective preventing a recurrence PCP.
Little formal training is provided in anesthesiology residency programs to help acquire, develop, and practice skills resource management decision making during crises practice. Using anesthesia crisis (ACRM) principles developed at another institution, 68 anesthesiologists 4 nurse-anesthetists participated an ACRM course held over a 2 half-month period. The environment was recreated real operating room, with standard equipment simulations requiring actual performance of clinical...
Four heroin addicts developed hematogenous osteomyelitis with indolent courses. In three, characteristic X-ray changes evolved during their hospital stays. Only one had a positive blood culture. The infecting organisms were not of those found in spontaneously occurring but rather reflected the spectrum pathogens that infect addicted to narcotics: staphylococci, Gram-negative rods, and yeasts. such patients early surgical exploration is necessary establish an accurate bacteriologic diagnosis.
Background Cardiovascular stability is an important prerequisite for any new volatile anesthetic. We compared echocardiographically derived indices of myocardial contractility during inhalation induction with sevoflurane and halothane in children. Methods Twenty children were randomized to receive either or anesthesia. No preoperative medications given. Myocardial was evaluated at baseline end-tidal concentrations 1.0 minimum alveolar concentration (MAC) 1.5 MAC. Results There no differences...
We examined the incidence of zoster in 346 children with underlying acute lymphoblastic leukemia who were immunized live attenuated varicella vaccine while remission. also compared a subset 84 these matched group had natural infection varicella. Of vaccinated children, 5 (1.45 percent) became infected after 10,878 months observation, for an 0.552 case per 100 person-years. Among pairs subjects, occurred 3 (3.6 subjects during 2936 observation--an 1.23 cases person-years--and 11 (13.1 4245...
The authors conducted a retrospective cohort study to determine the incidence, bacteriological features, and risk factors for postcraniotomy meningitis.Patients older than 18 years who underwent nonstereotactic craniotomies between January 1996 March 2000 survived more 7 days were included. Operations placement of burr holes shunts excluded. Records first 30 postoperative abstracted. Host factors, types craniotomy, pre- variables evaluated as meningitisAmong 453 patients, there 25 cases...
Abstract Nosocomial fungal pulmonary infections ( Zygomycetes, Aspergillus sp.) developed in two premature infants a special care unit (SCU) adjacent to an area of renovation. Inspection showed that inadequate barriers permitted the passage airborne particles between areas, and cultures confirmed significantly higher (p≤0.05) density mold spores SCU (0.88 cfu per hour settling plate) compared construction-free comparison (0.22 plate). The major source was dust above hospital's false ceiling....
Staphylococcus aureus organisms vary in the function of staphylococcal virulence regulator gene agr. To test for a relationship between agr and transmission S. aureus, we determined prevalence genetic basis dysfunction among nosocomial methicillin-resistant (MRSA) an area MRSA endemicity. Identical inactivating mutations were not detected epidemiologically unlinked clones within or hospitals. Additionally, most mutants had single mutations, indicating that they short lived. Collectively,...
To understand the factors associated with knowledge of non-occupational post-exposure prophylaxis (nPEP) and pre-exposure (PrEP), bathhouse patrons in New York City (NYC) were surveyed.554 men who have sex (MSM) at two NYC bathhouses given a standardised survey focused on nPEP PrEP time HIV testing. In previous 90 days, 63% respondents reported unprotected male partner 7% any known HIV-positive partner. Less than half having primary provider (primary care practitioner) was aware their MSM...
Abstract Non‐endemic Salmonella bacteremia tends to occur in patients with chronic disease. We reviewed all cases of infection documented adults at Bellevue Hospital during the years 1975–1982. Unexpectedly, most frequent underlying disease found among bacteremic was systemic lupus erythematosus (SLE). Patients SLE accounted for 6 30 bacteremias as compared 13 2,388 non‐ gram‐negative bacteremias. single isolate from blood patients. All were bacteremic. In contrast, isolates represented only...
Inactivating mutations in the Staphylococcus aureus virulence regulator agr are associated with worse outcomes bacteremic patients. However, whether dysfunction is primarily a cause or consequence of early bacteremia unknown. Analysis 158 paired S. clones from blood and nasal carriage sites individual patients revealed that recovery an agr-defective mutant was usually predicted by functionality isolates. Many agr-positive isolates produced low levels hemolytic toxins, but were similar to...
The current study identified bacterial factors that may improve management of methicillin-resistant Staphylococcus aureus (MRSA) nosocomial pneumonia. Isolates were obtained from 386 patients enrolled in a randomized, controlled antibiotic efficacy. screened for production virulence and vancomycin susceptibility. After adjustment host such as severity illness treatment modality, cytotoxic activity was strongly inversely associated with mortality; however, it had no effect on clinical cure....
