Eleven cases of community-acquired Pneumocystis carinii pneumonia occurred between 1979 and 1981 prompted clinical immunologic evaluation the patients. Four heroin addicts developed hematogenous osteomyelitis with indolent courses. In three, characteristic X-ray changes evolved during their hospital stays. Only one had a positive blood culture. The infecting organisms were not of those found in spontaneously occurring but rather reflected the spectrum pathogens that infect addicted to narcotics: staphylococci, Gram-negative rods, and yeasts. such patients early surgical exploration is necessary establish an accurate bacteriologic diagnosis.
