今回我々は肺腺癌と食道扁平上皮癌の同時性重複癌の9例を対象とし,術後合併症と予後について検討した.年齢中央値67歳,男性6例,女性3例.術式は右肺と食道の同時手術が5例,二期的手術が4例であった.肺癌切除術式は肺葉切除5例,区域切除4例.全例で完全切除を行い,術後在院死は認めなかった.同時手術例では2例(40%)に周術期合併症を認め,右下葉切除例が誤嚥性肺炎,胸骨後再建胃管の縫合不全,乳糜胸,気管支断端瘻および膿胸を併発した.S6区域切除例で肺炎およびARDSを併発し,気管支瘻をきたした.一方,二期的手術を行った4症例では,重篤な合併症は認めなかった.全生存期間における3年生存率は76.2%,生存期間中央値は67.1ヵ月であり,食道・肺ともに完全切除可能であれば,手術加療を考慮すべきと考えられた.また,右肺癌重複例で同時手術を行う際は,肺合併症と消化管縫合不全の同時併発に留意すべきである.
