症例:85歳男性.脱力を主訴に救急搬送され,脱水と胸部CTで右肺下葉に肺炎像を認め補液と抗菌薬による加療を開始した.第11病日に右胸水と右肺下葉に空洞影が出現し,胸水検査でグラム陽性桿菌の増殖を認め,第18病日にBacillus Cereusが検出された.胸腔ドレナージを行うも症状が改善せず,第20病日に開胸下右膿胸腔掻爬術および右下葉切除術を施行した.術中所見で右肺下葉は広範に壊死しておりB. Cereusによる肺化膿症に伴った急性膿胸と診断した.
