We report on a 55-year-old man who developed de novo DAVF in left ACC 5 years after curative transvenous embolization for right ACC. Angiography revealed that the lesion demonstrated more aggressive arteriovenous shunt flow than initial lesion. 症例は46歳の女性である.2013年6月に下顎リンパ節腫脹があり,明るい細胞質を有する中型リンパ球等の増生所見と免疫染色の結果から血管免疫芽球性T細胞リンパ腫(angioimmunoblastic T-cell lymphoma; AITL)と診断,化学療法を行い,同年12月に完全寛解と判断された.翌年5月から同じことを何度も聞くようになり,当科を受診した.記銘力障害,髄液細胞数・蛋白の上昇,MRIでの両側乳頭体や視床周囲の造影効果を伴うFLAIR高信号域を認めた.脳生検にてAITLの再発と診断した.AITLの頭蓋内再発は報告がないが,経過中に中枢神経症状を合併した場合はその可能性を考慮すべきである.

2015年に発表された5つのランダム化比較試験により, 機械的血栓回収療法の有効性が確立した. 本邦の適正使用指針では発症8時間以内が治療適応となるが, いわゆるwake up strokeやunknown onsetなどでは, その適応に悩む症例がある. われわれは発症から18時間経過した右内頚動脈塞栓症例において, diffusion weighted image (DWI)-fluid attenuated inversion recovery (FLAIR) mismatchの存在から機械的血栓回収療法を施行し神経症状の回復を得た症例を経験した. 本症例では右頚部内頚動脈から中大脳動脈にかけて連続する多量の血栓が存在し, 血栓の飛散が段階的に生じたことでtherapeutic time windowが延長された可能性も示唆された. 発症から時間が経過した症例においても, DWI-FLAIR mismatchが認められる症例では機械的血栓回収療法が有効となる可能性が示唆された.
