2015年に発表された5つのランダム化比較試験により, 機械的血栓回収療法の有効性が確立した. 本邦の適正使用指針では発症8時間以内が治療適応となるが, いわゆるwake up strokeやunknown onsetなどでは, その適応に悩む症例がある. われわれは発症から18時間経過した右内頚動脈塞栓症例において, diffusion weighted image (DWI)-fluid attenuated inversion recovery (FLAIR) mismatchの存在から機械的血栓回収療法を施行し神経症状の回復を得た症例を経験した. 本症例では右頚部内頚動脈から中大脳動脈にかけて連続する多量の血栓が存在し, 血栓の飛散が段階的に生じたことでtherapeutic time windowが延長された可能性も示唆された. 発症から時間が経過した症例においても, DWI-FLAIR mismatchが認められる症例では機械的血栓回収療法が有効となる可能性が示唆された.

脳動脈瘤の血管内治療後に,遅発性に発生する白質病変についての報告が散見される.本病態は,血管内留置デバイスから溶出するニッケルに対するアレルギー,あるいは治療中にカテーテルなどから剝離する親水コート剤に対する異物反応などが発生機序と推測されている.今回我々は,潰瘍性大腸炎を基礎疾患に有する内頚動脈瘤患者のステント併用コイル塞栓術後,遅発性に大脳白質の浮腫が発生し,病変縮退後に浮腫病変内に微小虚血巣の残存した1例を経験した.血管内治療後の遅発性白質病変とそのリスク要因について,考察を加え本症の啓発を行う.
