The endocapsular tension ring has helped in the management of patients with moderate loss zonular support. However, eye profound dialysis or weakness may require scleral fixation for centration and long-term stabilization. We used a new, modified designed to provide support without violating integrity capsular bag 4 patients. All had extreme preoperatively significant risk progressive weakness.
