A 59-year-old man, with a history of Roux-en-Y gastric bypass 9 years previously, presented chronic epigastric pain thought to be due recurrent marginal ulceration. On diagnostic endoscopy he was found have 3-cm pouch and 18-mm gastrojejunal anastomotic diameter. Additionally, 1-cm cratered ulceration on the jejunal aspect post-bypass anastomosis 3-mm fistulous opening in distal were noted. The presence bubbles when area flooded water suggested communication between remnant stomach.
