背景.肺の定型的カルチノイドでリンパ節転移を示す例は少ない.血清adrenocorticotropic hormone(ACTH),progastrin-releasing peptide(pro-GRP)高値で,縦隔リンパ節転移陽性の1例を経験したので報告する.症例.41歳,女性.ふらつきを主訴に当院内科を受診し,内分泌学的検索からCushing症候群と診断された.中葉に最大径11 mmの腫瘤を認め,異所性ACTH産生腫瘍が疑われ,確定診断目的で当科へ紹介された.術中迅速病理診断はカルチノイドで,リンパ節郭清(ND2a-1)を伴う中葉切除を施行した.最終病理診断は定型的カルチノイドで,右下部気管傍リンパ節(#4R)転移陽性,pT1aN2M0,p-stage IIIAであった.上昇していたACTH,pro-GRPは,術後に正常域となった.化学療法を追加し,術後20か月を経た現在,再発を認めていない.結論.肺定型的カルチノイドであってもリンパ転移陽性例があることを念頭に置く必要がある.
