歯の喪失に至る歯周病や齲蝕は,口腔内微生物によって形成されるプラークに起因している.健全な口腔内環境を維持するには,患者の意識改革を得た上でプラークコントロールが継続されることが必要であり,そのためには歯科医師のみならず歯科衛生士の役割が重要となる.しかしながら,良好なプラークコントロールを得るたあに歯科衛生士による口腔衛生指導がどの程度必要なのか,また目標とした口腔衛生状態が得られた後の管理期間に良好な状態がどの程度持続し,またどの程度の間隔で歯科衛生士による衛生指導が必要かなど,口腔衛生指導における達成目標や時間を意識した報告は少ない.近年,歯科領域において医療の質を評価・改善するシステムであるクリティカルパスが導入されつつある.適切なクリティカルパスが導入されることにより,医療の標準化,患者へのインフォームド・コンセントの改善などが期待される.今回,歯科衛生士による口腔衛生指導用クリティカルパス作成を目的として,本学附属病院保存治療科を受診した患者を対象に,歯科衛生士によるブラッシング指導の回数とプラーク・コントロール・レコード(PCR)の変化との関係について検討した.患者...

肝移植後C型肝炎に対するDirect Acting Antivirals(DAA)治療効果を検討した.Genotype1は29症例で,ダクラタスビル+アスナプレビル(DCV+ASV)5例,ソホスブビル+レジパスビル(SOF+LDV)25例(含DCV+ASV無効1例),Genotype2が2症例でSOF+リバビリン治療を行った.DCV+ASVは5例中4例で治療完遂,3例でSustained viral response(SVR)24を達成.SOF+LDVは全例SVR24を達成.移植後2カ月以内の肝炎再燃例も含まれているが問題なく治療完遂可能であった.Genotype2はSVR24を達成.SOF中心レジメンで100%のSVR24達成率であり,移植後早期においても問題なくウイルス駆除達成可能であった.C型肝硬変の肝移植適応評価においてC型肝炎のネガティブインパクトはなくなったと言っても過言ではない.

コルチコステロイド含有軟膏(「ス」軟)7製品, コルチコステロイド含有クリーム(「ス」クリーム)9製品, コルチコステロイド含有ゲル(「ス」ゲル)3製品, 非ステロイド抗炎症剤含有軟膏(非「ス」軟)2製品, 非ステロイド抗炎症剤含有クリーム(非「ス」クリーム)2製品の合計23製品, およびホリスタースティア, トリイアレルゲンのうちから外用剤に配合されている可能性の高いアレルゲン27種と基剤2種を114例の皮膚疾患患者にclosed patch testを施行した。Closed testの結果製品の(+)以上(本邦基準)陽性率は48時間判定, 72時間判定の順に, 「ス」軟1.5%, 0.8%, 「ス」クリーム1.8%, 2.0%, 「ス」ゲル9.2%, 15.1%, 非「ス」軟6.2%, 5.4%, 非「ス」クリーム6.6%, 10.6%あつた。アレルゲンの陽性率(ICDRG基準)は上位5位まで, ネオマイシン13.8%, アクリノール5.4%, レゾルシン3.4%, ラノリンアルコール3.3%, P. G. 3.3%, ホルマリン3.3%, メントール3.3%であつた。
