症例は70歳の女性で,2週間前から続く気分不良と嘔吐を主訴に当院救急外来を受診した.腹部造影CTにて小腸内に石灰化を伴う異物と,口側の小腸の拡張像を認め閉塞性イレウスと診断された.保存的治療で改善せず,手術目的に当科転科となった.腹部造影CTで胆囊底部にガス像を認め,胆囊頸部の結石,胆囊と十二指腸の近接も認めたことから胆囊十二指腸瘻に伴う胆石イレウスと診断した.初回は単孔式腹腔鏡下に腹腔内を検索し,胆囊周囲の著明な癒着と,結石の嵌頓した腸管を同定した.創部から体外に腸管を導出して結石の摘出のみ行った.33日後,腹腔鏡下に胆囊摘出術と胆囊十二指腸瘻閉鎖を行う二期的手術を施行した.胆摘後,十二指腸側の瘻孔部には大網を被覆した.術後経過は良好であった.近年,胆石イレウスに対する腹腔鏡下手術の有用性が報告されている.腹腔鏡下二期的手術を行い良好に経過した1例を経験したため報告する.

症例は49歳の男性で，約1年半前に，突然の腹痛を主訴に当院へ紹介された．腹部造影CTにて上腸間膜動脈解離を指摘され，同時に腸回転異常症に伴う中腸軸捻転の所見を認め，入院となった．保存的治療にて速やかに改善したため，外来にて経過観察されていた．今回，前日からの腹痛と嘔吐を主訴に当院を受診した．腹部造影CTにて中腸軸捻転の所見を認め，捻転部での通過障害が疑われた．腸管血流障害の所見を認めなかったため，保存的治療を行ったが改善せず，腹腔鏡下に中腸軸捻転の解除術と虫垂切除術を施行した．Treitz靭帯は無形成で，上腸間膜動脈の腹側に十二指腸，背側に横行結腸を認めるreversed rotationの腸回転異常症であった．術後経過は良好であった．成人腸回転異常症に伴う中腸軸捻転に対する腹腔鏡下手術は，腸管血流障害の危険がなく，十分な術前精査が可能である症例に対しては有用であると考えられる．
