A 27-year-old woman with progressive diaphyseal dysplasia (Camurati-Engelmann disease) received pamidronate and corticosteroid therapy for bone pain. During therapy, disease activity was assessed serially using scintigraphy Tc-99m HMDP. With administration, the pain became worse uptake of HMDP increased, whereas administration improved reduced uptake. In this case, produced different effects. Bone allowed an objective assessment response to these treatments, accurately reflecting clinical symptoms.

Patients with ankylosing spondylitis (AS) are prone to spinal fracture after even minor trauma. We report a case of thoracic in patient AS who developed secondary neurological deficit due delayed diagnosis and prolonged conservative treatment. When the occurred, fractured segment showed no displacement, but epidural hematoma was present. Surgical treatment produced significant improvement, although incomplete paralysis persisted.
