距骨外側突起骨折は比較的稀な骨折であり,初診時に見落とされることが多い.今回我々は,足関節外果剥離骨折を伴った距骨外側突起骨折を経験したので報告する.症例は67歳女性.平成17年4月,旅行中に階段を踏みはずし左足関節を背屈強制され受傷.同日近医受診.左足関節外果剥離骨折の診断にてシーネ固定され,2日後当院紹介受診.左足関節外果周囲の腫脹,疼痛,外果前面ならびに後面の圧痛を認めた.初診時単純 X 線像では,左足関節外果に剥離骨片を認めた.CT 再構築像にて距骨外側突起骨折も明らかとなった.後日外果部ならびに外側突起部を cancellous screw で整復固定した.術後3週目よりサポーター装着下にて部分荷重歩行を開始し,術後6週で1本杖歩行にて退院した.1年後抜釘し,経過良好である.距骨外側突起骨折は比較的稀な骨折で,しばしば見落とされる.今回我々が経験した症例は,診断に CT が有用であった.
