Autogenous bone grafts from the ilium are frequently harvested for purposes of union and/or stability. Although some donor site complications may be unavoidable, awareness anatomy and aid in planning approach minimizing risks. Documented include pain, nerve arterial injury, peritoneal perforation, sacroiliac joint instability, herniation abdominal contents through defects ilium. Strict observation relevant anatomic considerations will help avoiding these complications.
In AIDS research, relatively little attention has been paid to reliability of self-report drug users. The authors examined the test-retest Risk Behavior Assessment (NIDA, 1991) questionnaire. This structured-interview questionnaire was administered twice 196 users in 5 cities over a 48-hr period. Findings indicated that respondents consistently use, injection practices, and sexual behaviors; discrepancies do not appear reflect systematic decreases or increases self-report; unreliability is...
Context: Contingency management interventions that provide tangible incentives based on objective indicators of drug abstinence are efficacious in improving outcomes substance abusers, but these treatments have rarely been implemented community-based settings. Objective: To evaluate the efficacy an abstinencebased contingency intervention as addition to usual care community treatment settings.Design: Random assignment or plus abstinence-based for 12 weeks.Setting: Eight outpatient...
Synovial fluid biomarkers have demonstrated diagnostic accuracy surpassing the currently used tests for periprosthetic joint infection (PJI).The purpose of this study is to directly compare sensitivity and specificity synovial α-defensin immunoassay leukocyte esterase (LE) colorimetric test strip.Synovial was collected from 46 patients meeting inclusion criteria prospective study. samples were tested with both a novel synovial-fluid-optimized LE strip. The Musculoskeletal Infection Society...
This study examined the reliability and validity of Risk Behavior Assessment, a structured interview questionnaire designed to evaluate drug use sexual HIV risk behaviors. Participants were 218 users currently not in treatment who completed RBA two times over 48-hour period gave urine samples on both occasions. We internal consistency test-retest found that, overall, reliably report behavior, although reports specific needle practice behavior items was somewhat lower. Validity results...
Patellofemoral pain due to an unusual suprapatellar fibrous nodule developed in three patients after posterior stabilized total knee arthroplasty. Each patient had a catch or clunk associated with on extension of the knee. At revision, prominent was found at junction proximal patellar pole and quadriceps tendon. The pathogenesis this may involve impingement anterosuperior edge intercondylar notch area femoral component into tendon prosthesis itself Roentgenographic examination revealed...
Synovial fluid aspirates from 50 patients with symptoms after total knee arthroplasty were analyzed by means of the polymerase chain reaction for presence bacterial deoxyribonucleic acid indicative infection. specimens processed using a rapid lysis and extraction protocol, subjected to amplification universal primers, products hybridization methodology. Polymerase testing on preoperative yielded 32 positive Standard microbiologic culturing assays performed same samples gave 6 infection...
We have previously described a unique gene expression signature exhibited by synovial fluid leukocytes in response to bacterial infection, identifying number of potential biomarkers for infection. However, the diagnostic performance these an immunoassay format is unknown. We therefore evaluated sensitivity, specificity, and accuracy several compared them current standards testing periprosthetic infection. We prospectively collected from 14 patients classified as having infection 37 aseptic...
An antibiotic-impregnated polymethylmethacrylate spacer block functions in the delayed exchange approach to chronic deep infection of total knee arthroplasty. It provides antibiotic delivery, greater patient comfort and mobility, temporized reimplantation or other surgical options, improved soft-tissue healing tension. Bone quality is enhanced, opportunity prepare for custom implants special situations preserved, a more facile can be anticipated.
The alpha-defensin test has been previously demonstrated to be highly accurate in the diagnosis of prosthetic joint infection (PJI), nearly matching Musculoskeletal Infection Society definition for PJI. However, relationship between levels and differing infecting organism not yet investigated. The purpose this study is describe breadth organisms that can trigger a positive synovial fluid result setting PJI also assess magnitude terms various pathogen characteristics.Between December 2012...

Forty knees in 40 patients who had a chronic extensor mechanism disruption after knee arthroplasty underwent allograft placement to restore function. Thirteen were infected previously and 11 did not respond previous attempts at direct repair or reconstruction. Two died two above the amputation because of recurrent infection. The final group 36 was studied for mean followup 3.6 years (range, 2-10 years). Clinical evaluations performed using modified Knee Society scoring system. average range...
In 20 patients with patellar "clunks" after posterior stabilized total knee arthroplasty, the average time to presentation was 10.7 months postarthroplasty. All demonstrated an audible and often painful "clunk" during extension. Fourteen procedures (11 arthroscopic debridements three component revisions) were performed in 12 patients. At reoperation, a suprapatellar fibrous nodule seen wedge into intercondylar notch flexion dislodge extension, generating symptoms. The disorder resolved...
The three primary objectives of this study were (1) to document the prevalence hepatitis C virus (HCV) and awareness one's status in a sample street-recruited injection drug users (IDUs); (2) compare use HCV risk behaviors IDUs who reported that they infected with those unaware their subsequently tested positive for antibodies; (3) assess associations between factors by comparing negative.IDUs responded structured interview questions assessing offered free test.Subjects recruited through...
