This case shows the risk of severe cardiovascular complications following lumbar spine surgery, with progressive high output heart failure caused by an iatrogenic iliac arteriovenous fistula. Careful history taking and thorough physical examination are essential in guiding diagnosis. Endovascular repair can provide excellent short- long-term outcomes.
