A 66-year old woman had a left breast mass. Biopsy showed invasive ductal carcinoma. PET/CT scan demonstrated hypermetabolism in the and atypical heterogeneously increased uptake throughout skeleton as well minimally FDG-avid right adrenal myelolipoma. 4 months later after 6 cycles of neoadjuvant chemotherapy size FDG avidity this mass concerning for metastasis uniformly skeletal avidity. Pancreatic cancer is often detected in late stages, which contributes to its grim prognosis. Although the manifestations of pancreatic most include nonspecific gastrointestinal complaints, we report a case with sole initial complaint halitosis and subsequent diagnostic workup demonstrating mass secondary pancreatocolonic fistulization.
