Idiopathic pulmonary hemosiderosis is characterized by repeated alveolar hemorrhaging. We herein report a 52-year-old Japanese woman who had shortness of breath, diffuse small nodules, thin-walled cysts, and bronchiolectasis. 背景.肺癌回腸転移は生前診断されることは稀である.症例.症例1は61歳男性.非小細胞癌の術後再発で,化学療法中に急性腹症を発症した.胸部X線,腹部CTにて腹腔内にfree airを認め,消化管穿孔と診断した.緊急手術を行ったところ,回腸に腫瘍を認め,肺癌の回腸転移による消化管穿孔と診断した.術後3日目に敗血症で死亡した.症例2は72歳男性.非小細胞癌(cT3N2M1,stage IV)で,化学療法施行中に急性腹症と大量下血を認めた.保存的治療では症状が改善せず,開腹手術を行ったところ回腸に腫瘍を認め,肺癌回腸転移からの出血と診断した.術後1カ月で癌死した.結論.今後,急性腹症を生じ緊急処置を要する可能性のある小腸転移の早期発見を目的としたFDG-PETやPET-CT検査によるスクリーニング,さらに消化管に集積を認めた症例には,積極的な消化管内視鏡検査による診断が必要であると考えられた.
