小児特発性ネフローゼ症候群 (NS) では大部分の症例がグルココルチコイド療法により寛解するが,NSの発症病理に関するグルココルチコイドの役割は十分に解明されておらず,また治療抵抗性を認める症例の原因も明らかではない。我々はNS患者を中心にステロイド感受性に関するグルココルチコイドの分子作用機序についてその遺伝子転写調節の細胞内メッセンジャーであるグルココルチコイドレセプター (GR) とその発現制御因子についての解析を行った。その結果,頻回再発型ならびにステロイド抵抗性ネフローゼ症候群にGR発現量の低下を高頻度に認め,これらの症例ではGR Down regulationによるステロイド依存性転写調節障害が示唆された。またレセプタ一結合蛋白である熱ショック蛋白はGR発現量と相関することよりグルココルチコイドーGR系の情報伝達が阻害されステロイド抵抗性の原因となることが示唆された。
