三叉神経の機械的刺激により徐脈, 血圧低下や心停止を起こすことがあり, 三叉迷走神経反射と呼ばれる. 今回われわれはテント上未破裂脳動脈瘤クリッピング術中に, 硬膜の刺激による三叉迷走神経反射が原因と思われる心停止をきたした症例を2例経験したので報告する. 代表症例は63歳の女性で, 未破裂動脈瘤に対して開頭クリッピング術を施行した. クリップを脳動脈瘤に近づけた際に, クリップが硬膜に接触したところ4∼5秒間の心停止を2回きたした. 術中操作の中断ですぐに回復し, 手術後の経過は問題なかった. 硬膜刺激による三叉迷走神経反射はすべての開頭手術で起こる可能性があり, テント上未破裂脳動脈瘤クリッピング術でもその頻度は低くない. 生じた場合はすみやかに術中操作を中断することが最も確実な対処法である.
