本論文は,転移を有する膵癌でgemcitabine(GEM)化学療法後に治癒切除ができた2例の報告である.症例1は69歳男性.CTで膵体部腫瘍を指摘され,EUS-FNAでadenocarcinomaの診断となった.開腹手術に臨んだが,腹膜播種結節を認め試験開腹術となった.GEM単剤化学療法を開始し,18か月間増大なく播種結節は同定し得なかった.再度開腹手術に臨み腹膜播種は認めず,膵体尾部切除術を施行した.症例2は73歳女性.CTで膵体部腫瘍と肝S4に低吸収域を認め,EUS-FNAでadenocarcinomaであり,膵癌肝転移の診断となった.GEM単剤化学療法を開始し,原発巣,肝転移巣は不明瞭化し,9か月間増悪なく,腹腔動脈合併膵体尾部切除術,肝部分切除術を施行した.切除不能膵癌に対するconversion surgeryの適応を検討する上で貴重な症例であり,文献的考察を加え報告する.
