ABSTRACT Chylothorax is the accumulation of chyle in pleural cavity. Tuberculosis one its rarest causes. Diagnosis chylothorax can be made by evaluation triglycerides concentration fluid. It primarily managed medically but surgery indicated when patients fail to improve after a trial medical management. Here, we present case 28‐year man who developed while undergoing anti‐tuberculous treatment (ATT). After failing initial management, he underwent thoracic duct ligation with good result.
