臨床所見・画像から何を考えるか?症例は Hunt and Kosnik grade Ⅱ,Fisher 分類 group 3 の 83 歳のくも膜下出血(SAH)患者である(Fig. 1) . 発症前は外出や家事などもされていた元気な方であっ た.2 日前にも同様の症状があり,脳動脈瘤の再破裂を 疑う.SAH 発症以前から右眼を失明している.CT では 右後頭葉に陳旧性脳梗塞を認める.3D computed tomography angiography(CTA)では脳底動脈-左上小脳動脈 (BA-SCA)分岐部に左上方に突出して先端部分に bleb を伴う動脈瘤を認める (Fig. 2, 3) .瘤の neck 径は 5 mm であり,dome/neck ratio=1.27で,後床突起から までの高さは 7 である.右椎骨動脈(VA)に 70% 程度の狭窄を認める(Fig.2A) .胸部 X 線で大動脈弓の 石灰化を認める.
