Dasatinib is the second-generation tyrosine kinase inhibitor used in treatment of chronic myeloid leukemia. Proteinuria has been reported with this agent. We describe two kidney biopsy–proven cases dasatinib-induced thrombotic microangiopathy that responded to stoppage dasatinib and using an alternate inhibitor. Certain specific inhibitors lead endothelial injury renal-limited microangiopathy. Hematologists nephrologists need be familiar off-target effect dasatinib.
