In prednisone-dependent severe asthma, uncontrolled sputum eosinophilia is associated with increased numbers of group 2 innate lymphoid cells (ILC2s). These represent a relatively steroid-insensitive source interleukin-5 (IL-5) and IL-13 are considered critical drivers asthma pathology. The abundance ILC subgroups in neutrophilic or mixed granulocytic (both eosinophilic neutrophilic) airway inflammation, prone to recurrent infective exacerbations, remains unclear. Here, we found by flow...
Background In clinical trials, the two anti-interleukin (IL)-5 monoclonal antibodies (mAbs: mepolizumab and reslizumab) approved to treat severe eosinophilic asthma reduce exacerbations by ∼50–60%. Objective To observe response anti-IL-5 mAbs in a real-life setting, evaluate predictors of suboptimal response. Methods four Canadian academic centres, predefined end-points 250 carefully characterised moderate-to-severe asthmatic patients were collected prospectively assess mAbs. Suboptimal was...
BackgroundWe previously showed in patients with poorly controlled eosinophilic asthma that a single dose of benralizumab resulted significantly improved Asthma Control Questionnaire (ACQ-6) score and 129Xe MRI ventilation defect percent (VDP) 28 days postinjection, VDP CT airway mucus occlusions were shown to independently predict this early ACQ-6 response benralizumab.Research QuestionDo responses at persist, do FEV1, fractional exhaled nitric oxide, plug improve during 2.5 year treatment...
Background Chronic cough significantly impacts individuals' quality of life and poses challenges for healthcare providers due to limited licensed treatments, side effects available medications, difficulty accessing non-pharmacologic interventions. Understanding priorities research, education, knowledge dissemination from the perspectives individuals with chronic can guide future efforts. This study aimed identify these key in Canada. Study design methods We conducted a cross-sectional using...
ABSTRACT Background In asthma, fluticasone-furoate/umeclidinium/vilanterol (FF/UMEC/VI) was previously demonstrated to improve FEV 1 and asthma-control, but the mechanism by which this occurs, especially at level of small-airways, is not well-understood. We asked question, does FF/UMEC/VI influence asthma-control improving airway structure-function occlusions? Hence, here our objective measure using 129 Xe MRI ventilation-defect-percent (VDP), CT airwave-oscillometry after 6-weeks...
Abstract Background Excess pulmonary iron has been implicated in the pathogenesis of lung disease, including asthma and COPD. An association between higher content sputum macrophages infective exacerbations COPD previously demonstrated. Objectives To assess mechanisms macrophage sequestration, test effect iron-loading on cellular immune function, prospectively determine if hemosiderin index can predict infectious Methods Intra- extracellular was measured cell-line-derived freshly isolated...
The minimal clinically important difference (MCID) and upper limit of normal (ULN) for MRI ventilation defect percent (VDP) were previously reported hyperpolarized 3He gas MRI. Hyperpolarized 129Xe VDP is more sensitive to airway dysfunction than 3He, therefore the objective this study was determine ULN MCID in healthy asthma participants. We retrospectively evaluated participants who underwent spirometry 129XeMRI on a single visit; with completed control questionnaire (ACQ-7). estimated...
Abstract Rationale On Wright-stained sputum cytospins, eosinophil differential of ≥ 1.2% is considered abnormal, and 2.3% identifies an eosinophilic endotype. We hypothesized that failure to consider free granules (FEG), the re-emergence (unmasking) eosinophilia due various reasons underestimate prevalence Methods This a retrospective analysis our Institutional Review Board-approved clinical database. Of 24,176 examinations sputa from patients with airway diseases, 17,693 were viable cell...
Despite Choosing Wisely recommendations for single unit red blood cell transfusion orders, ~50% of orders on the oncology ward at London Health Sciences Centre (LHSC) were two units. The LHSC is a 60 bed tertiary care unit. In mid 2016, was 18 months into its implementation computerised provider order entry (CPOE).By December 2017, increase proportion one-unit from 50% to 80.Outcome: % (aggregated monthly). Our initial theory that unawareness guidelines (established in 2014) and subscription...
Bronchopulmonary dysplasia (BPD) is an emerging risk factor for chronic obstructive pulmonary disease. For BPD survivors, there are no guidelines the management of lung disease that often misdiagnosed as asthma. Pulmonary magnetic resonsance imaging (MRI) provides clinically-relevant biomarkers ventilation abnormalities and emphysema. Here our objective was to quantify MRI in adults with understand underlying pathophysiologies responsible their symptoms abnormal pulmonary-function. We...
Large and small airway-wall lumen abnormalities occlusion are hallmark findings in asthma, but pathologic have also been reported the pulmonary vasculature. 129Xe MR-spectroscopy provides a way to quantify gas airways as it diffuses through alveolar tissue-membrane binds red-blood-cells capillaries. Static dynamic were undertaken participants with poorly controlled asthma whom there was significantly different MR RBC:membrane ratio compared healthy-volunteers. Cardiopulmonary oscillations...
It was previously shown that 129Xe MR ventilation images contain embedded texture features which help predict (before treatment initiated) those severe asthma patients who were more likely to experience early response anti-IL-5Rα biologic therapy. Thus, we postulated such would also identify with an enduring, late response. Here identified specific MRI predicted both and anti-IL-5 therapy, superior clinical measurements. These promising results suggest sensitively therapy interventions.
