症例は46歳男性．検診で胸部レントゲン異常を指摘され当院受診となった．胸部CTで右肺上葉に23 mm大の境界明瞭な結節を認めた．結節に対して気管支鏡を施行したが確定診断に至らず，FDG-PETでも有意な集積は認めなかったため経過観察されていた．その後，結節は約2年6ヵ月で35 mm大と徐々に増大傾向を示したため，胸腔鏡下右肺上葉切除術＋リンパ節郭清（ND2a-1）を施行した．組織学的所見上，腫瘍は不規則な紡錘形ないし不整形細胞の充実性増殖を呈していた．免疫染色ではSTAT6等で陽性を示し，肺内solitary fibrous tumorと診断された．さらに，腫瘍は細胞密度が高く，多数の核分裂（＞4 mitoses/2 mm2）を認めたため，病理学的に悪性と判定された．FDG-PETで有意な集積を認めなくても，増大傾向を示す腫瘍には，偽陰性例の存在を念頭におき手術を考慮すべきである．

背景．癌性胸膜炎による胸水貯留は胸腔ドレナージ施行後も肺が十分に再膨張しないことがあり，そのコントロールに難渋することがある．今回，我々は癌性胸膜炎による膨張不全肺に対して，ウロキナーゼの胸腔内注入を行い良好な肺の再膨張を得られた症例を経験したので報告する．症例．56歳女性．左乳癌術後経過観察中に左胸水の出現を認めた．その後胸水の著明な増加と労作時の呼吸困難が出現したため，当院乳腺科入院となった．胸腔ドレナージの後に胸膜癒着術が施行されたが，肺の膨張は不十分で胸腔内に多房性の胸水貯留が認められた．フィブリン溶解を目的としてウロキナーゼが胸腔内に投与された．その結果，形成されていた多房性の胸水は消失し良好な肺の再膨張が得られた．その後，再度胸膜癒着術が施行された．患者は約7ヶ月後に癌死となったが，その間胸水の再貯留を認めることなく安定した呼吸状態を保ち，自宅での生活が可能であった．結論．癌性胸膜炎に対するウロキナーゼの胸腔内注入は，trapped lungに対する治療法として有用であった．

非小細胞肺癌手術での術中胸腔内洗浄細胞診（Pleural lavage cytology：PLC）は予後因子として重要な役割を果たす．しかし，その洗浄手技は確立されておらず，一定の方法がない．今回，PLCを施行する際に使用する洗浄液の量に関しての検討を行った．対象は2008年11月から2011年5月までの非小細胞肺癌手術症例で，開胸直後に生理食塩水を洗浄液として用い，5 ml, 100 mlの順でPLCを行った．それぞれの洗浄液中の腫瘍細胞の検出率を検討した．期間中のPLC施行例は435例，腫瘍細胞陽性例は40例（9.2%）であった．そのうち5 mlの洗浄液で腫瘍細胞を認めた症例は36例（8.3%），100 mlの洗浄液では38例（8.7％）に認め，検出率に差は認めなかった（κ係数=0.9114）．PLCにおいて使用する洗浄液の量は少量でも検出率を損なうことなく施行しうると考えられた．
