症例1:77歳の男性,胸部中部食道癌 T1bN0M0 cStage IAに対し根治的化学放射線療法(CRT)を施行,complete response(CR)を得た.4年後の上部消化管内視鏡検査(EGD)にて局所再発を認め,光線力学療法(photodynamic therapy:PDT)を施行.その2カ月後に再発を認め,右開胸開腹食道亜全摘を施行した.術後3年8カ月経過するが無再発生存中である.症例2:56歳の男性,胸部中部食道癌 Stage IAに対し化学放射線療法を施行,CRを得た.CRT後5年のEGDにて局所再発を認め,内視鏡的粘膜剥離術,PDTを同時に施行した.1年6カ月後に局所再発を認め,右開胸開腹食道亜全摘を施行した.術後3カ月経過するが無再発生存中である.PDT後の局所遺残再発に対する食道切除術を行い良好な経過を得た2症例を経験したので報告する.
われわれは頸部食道癌術後外側咽頭後リンパ節(Rouviere)転移症例4例を経験した.頸部食道癌に対する初回治療は手術が3例,化学放射線療法が1例であった.症例1:術後2カ月目にRouviere転移を認め化学放射線療法を行ったが,奏効せず術後7カ月目に死亡した.症例2:術後2カ月目にRouviere単独再発をきたし,化学療法にて完全奏効が得られ術後5年生存中である.症例3:術後43カ月目に右Rouviere単独再発をきたしCRTを行い,完全奏効が得られ術後5年生存中である.症例4:初回治療のCRT後8カ月目に両側Rouviere再発をきたしサルベージ手術を施行したが,切除困難であり初回CRT後19カ月目に死亡した.頸部食道癌においてRouviereは領域リンパ節として選定されてはおらず遠隔転移として考慮される病変であるが,自験例4例中2例は治療が奏効し,長期予後が得られており,早期の再発診断,治療を行うことが重要であると考えられた.
