目的 探讨高位旋转点的腓动脉穿支蒂皮瓣修复跟骨骨折术后钢板外露的可行性及临床疗效。 方法 回顾性分析2010年5月—2013年8月保定市第一中心医院骨科收治的21例跟骨骨折术后出现钢板及跟骨外露患者的临床资料。21例患者均行腓动脉穿支蒂皮瓣修复创面,皮瓣旋转点选择外踝上7~9 cm处的腓动脉外踝上第2穿支,术后观察皮瓣成活率及皮瓣感觉恢复情况。 结果 21例患者手术均顺利完成,皮瓣均成活。全组患者随访6~12个月(平均9.1个月),皮瓣血运良好,质地柔软、耐磨,伤口一期愈合,无骨感染发生。其中12例皮瓣经过二期修整后外形美观,满足穿鞋等要求,感觉恢复达S2级;3例行腓肠外侧皮神经移位的患者,皮瓣感觉恢复达S3级,但无明确的两点分辨觉。 结论 高位旋转点的腓动脉穿支蒂皮瓣可用于修复跟骨骨折术后钢板外露创面,其手术方式安全、可靠,皮瓣修薄后外形美观,适合穿鞋袜等日常要求,可一定程度恢复浅感觉。
