右胃大網動脈グラフトによる冠動脈バイパス術後の膵頭十二指腸切除術を経験したので報告する.症例は67歳,男性.1年前に陳旧性心筋梗塞を発症し3枝病変に対して当院心臓血管外科で右胃大網動脈グラフトによる冠動脈バイパス術が施行された.術後経過観察で腹部造影CTを撮像したところ,肝内胆管の拡張を指摘され,当科紹介となった.下部胆管癌の診断で,胃十二指腸動脈,右胃大網動脈による亜全胃温存膵頭十二指腸切除術を施行した.術中,グラフトの血流はドップラーで確認しつつ,グラフトの背側で胃空腸吻合を行った結果,グラフト血流低下を防ぎ得た.術後もグラフトの血流は良好であった.術後膵液瘻が発生したが,保存的治療で改善し,術後34日目に退院となった.

症例は56歳，男性．胆嚢結石症に対して腹腔鏡下胆嚢摘出術を施行した．術後3日目以降も心窩部痛と発熱が持続し，TAZ/PIPCを投与したが改善せず，CTおよび腹部超音波検査で感染性肝嚢胞と診断した．術後10日目に超音波誘導下に経皮経肝膿瘍ドレナージを施行し，白色膿性排液を50ml吸引し，ドレーンを留置した．嚢胞内容液および血液の細菌培養検査では有意な病原体の発育は認めなかった．ドレナージ後は胆汁流出もなく速やかに症状は改善し，術後24日目に退院となった．肝嚢胞は日常の診療で遭遇する機会が多く，無症状で経過することがほとんどであるが，稀に手術後に感染をきたすという報告がある．自験例では，胆嚢に手術操作が加わることによって，胆嚢炎の起炎菌が周囲の静脈を介して経門脈的に肝嚢胞に波及したと推察した．胆嚢炎を併発している患者の胆嚢摘出後には，肝嚢胞の感染の発症を考慮しておく必要があると思われた．
