There is minimal information on coronavirus disease 2019 (COVID-19) in immunocompromised individuals. We have studied 10 patients treated at 12 adult care hospitals. Ten kidney transplant recipients tested positive for severe acute respiratory syndrome 2 (SARS-CoV-2) by polymerase chain reaction, and 9 were admitted. The median age was 57 (interquartile range [IQR] 47-67), 60% male, 40% Caucasian, 30% Black/African American. Median time from to COVID-19 testing 2822 days (IQR 1272-4592)....
We compared the outcome of COVID-19 in immunosuppressed solid organ transplant (SOT) patients to a naïve population. In total, 10 356 adult hospital admissions for from March 1, 2020 April 27, were analyzed. Data collected on demographics, baseline clinical conditions, medications, immunosuppression, and course. Primary was combined death or mechanical ventilation. assessed association between primary prognostic variables using bivariate multivariate regression models. also endpoint SOT an...
Tacrolimus, a calcineurin inhibitor widely used in transplant immunosuppression, requires careful monitoring due to its narrow therapeutic index and metabolism by cytochrome P450 CYP3A4. We present case of 72-year-old kidney recipient who developed acute tacrolimus toxicity following treatment with nirmatrelvir/ritonavir (Paxlovid) for COVID-19. The patient presented altered mental status, injury, supratherapeutic levels (>90 ng/mL). Given persistent toxicity, was held, intravenous phenytoin...
Abstract Background Immunosuppressive therapies proposed for Coronavirus disease 2019 (COVID-19) management may predispose to secondary infections. We evaluated the association of immunosuppressive with bloodstream-infections (BSIs) in hospitalized COVID-19 patients. Methods This was an institutional review board-approved retrospective, multicenter, cohort study adults over a 5-month period. obtained clinical, microbiologic and laboratory data from electronic medical records....
Summary: Pyoderma gangrenosum (PG) is an inflammatory disease characterized by sterile infiltration of the skin neutrophils. We describe a case 63-year-old woman who developed PG following abdominal wall reconstruction. Her initial presentation was thought to be consistent with surgical site infection. Antibiotic therapy initiated, and patient taken for multiple irrigation/lavage her abdomen debridement necrotic tissue. Wound cultures remained negative, maximal antibiotic coverage did not...
Vaccination with the inactivated influenza vaccine is routinely recommended for all patients before and after transplant, reduction in complications noted transplant recipients. The relatively well tolerated few mild side effects. Cytomegalovirus (CMV) infection can reactivate both solid organ hematopoietic stem cell recipients, some progressing to disease. There are multiple factors known contribute reactivation subsequent CMV disease, however vaccination has not been reported as a specific...
Historically, increases in blood culture (BC) fill volumes (FVs) have been shown to increase yield of BCs and lower contamination rates. Low FV are a common cause false negative BCs. 10 mL is considered an ideal for BC. In 2015 2016, at North Shore University Hospital, FVs averaged <5 per 2017, several interventions were implemented FVs, including convening informal meetings seminars educate nursing staff, educational phlebotomy posters, placing 10-mL markings on BC bottles using butterfly...
We describe a case of 56 year-old male who presented to the hospital with pneumonia and bacteremia secondary Streptococcus pneumoniae. After admission initiation appropriate antibacterial therapy he began complain scrotal pain. Ultrasound testes confirmed findings left-sided epididymitis. Urine culture, urine gonorrhea chlamydia nucleic acid amplification HIV testing were negative. Clinical picture suggested seeding epididymis pneumoniae resulting infection.
