To evaluate a Geriatric Emergency Department Intervention (GEDI) model of service delivery for adults aged 70 years and older. A pragmatic trial the GEDI using pre-post design. is nurse-led, physician-championed, (ED) intervention; developed to improve care frail older in ED. The nurses had gerontology experience education provided targeted geriatric assessment streamlining care. final format included 2.4 full time equivalent working 7 days from 0700 h 1730 (1530 at weekends). A total of 106 pairs identical twins, whom 56 were concordant and 50 discordant for insulin-dependent diabetes, typed HLA-DR. In both the groups there was a high prevalence antigens DR3 DR4, low DR5 DR7, virtual absence DR2. The heterozygous phenotype DR3,DR4 more prevalent in than pairs. This therefore first demonstration genetic difference between twin These findings suggest that possession DR4 confers greater predisposition to diabetes does either antigen alone.

Twenty two out of 86 insulin dependent diabetics had serum C4 concentrations below the normal range. None 41 non-insulin tested low concentrations. Low values were seen in diabetes irrespective duration disease and did not appear to correlate with complement activation. There was a close correlation between identical cotwins, even when only one diabetic. These results suggest that concentration is an inherited phenomenon may predispose towards development diabetes.

Activation of T lymphocytes and islet cell antibodies were studied in two groups insulin dependent diabetics their non-diabetic identical cotwins. Group 1 comprised 12 "short term" twin pairs (diabetic diagnosed less than five years previously) whom only a third the cotwins likely to develop diabetes; 10 showed increased values activated lymphocytes, antibodies, or both. 2 "long more 11 none was these selected because all had shown at some time past, but still did so (one with an value...

Autistic students experience elevated rates of school exclusion, whether this be through expulsion, suspension, informal refusal enrolment, or refusal. This exclusion can come about from a range factors, including sensory cognitive overload, lack training in neurodiversity for teachers, an absent sense safety by the student at school, poor attitudes towards inclusion autistic schools. The impacts on parents logistical, financial, and psychological, with mental health outcomes. Australian...

Transgender and gender diverse youth (TGD youth; i.e., children adolescents who do not identify with their birth-assigned sex) face a variety of traumas adversities, including those explicit to identity and/or expression (hereafter “gender”; e.g., gender-related victimization, caregiver rejection). However, few studies or clinical assessment measures capture the full spectrum adversities TGD experience. A comprehensive examination gender- non-gender-related faced by is critical understand...

To determine whether the genetic predisposition towards Type 2 diabetes was associated with a defect in either islet‐cell function or insulin action, 12 non‐diabetic offspring each of whose parents both had were studied, together control subjects matched for age, sex, and weight. Fasting plasma glucose higher (5.5 ± 0.1 mmol I −1 (mean SE)) than controls (5.1 ) ( p < 0.05). Using an IVGTT sensitivity not significantly lower compared their (3.1 0.5 vs 3.8 1.0 min ***mU 10 −4 ). There no...

Emergency departments are chaotic environments in which complex, frail older persons living the community and residential aged care facilities sometimes subjected to prolonged emergency department lengths of stay, excessive tests iatrogenic complications. Given ageing population, importance providing appropriate, quality health for this cohort is paramount. One possible solution, a nurse-led, physician-championed, gerontological intervention team, provides frontload assessment, early...

Despite legislative and policy mechanisms that promote inclusion in education for children with disabilities, many students still experience exclusion from the setting. For example, autistic are more likely than their neurotypical counterparts to be expelled or suspended school refuse attend on an ongoing basis. This situation gives rise various challenges parents. discursive paper synthesizes current knowledge involving children, including causes consequences of exclusionary practices. The...

During the preclinical period of human insulindependent diabetes, both impaired pancreatic β-cell function and increased insulin resistance are found, although normoglycemia is preserved. To better understand changes in sensitivity that occur we performed a panel vivoβ-cell tests measured adolescent male baboons normal health after small dose streptozocin which did not induce hyperglycemia. Nine animals were studied before (stage 1) 1 week receiving low 2). There was no change fasting plasma...
