Two regimens for infusing vancomycin over 1 h (500 mg every 6 five doses or 100 12 three doses) were used in 11 volunteers. Subjects received both one week apart; the regimen first each subject was randomized. Nine receiving 1000-mg dose experienced "red-man (neck)" syndrome; none had reaction while 500-mg (P = .002). Plasma histamine concentration, measured 10 min during infusion of regimen, increased most subjects given doses; there only a slight change levels after doses. There...
A pharmacokinetic comparison of the two recommended dosages vancomycin given as multiple doses has not been previously performed. Eleven adult subjects with normal renal function randomly received 500 mg every 6 h (five doses) and, later, 1,000 12 (three doses). Each dose was infused over 1 h, and regimens were separated by week. Compared two-compartment fit, a three-compartment fit significantly reduced residual weighted sums squares. Accumulation occurred for both after repeated dosing...
The role of radiation therapy (RT) following breast-conserving surgery (BCS) in ductal carcinoma situ (DCIS) remains controversial. Trials have not identified a low-risk cohort, based on clinicopathologic features, who do benefit from RT. A biosignature (DCISionRT®) that evaluates recurrence risk has been developed and validated. We evaluated the impact DCISionRT clinicians' recommendations for adjuvant RT.The PREDICT study is prospective, multi-institutional, observational registry which...
The influence of multiple doses ciprofloxacin on the disposition caffeine and its major metabolite, paraxanthine, was investigated in healthy volunteers. Ten xanthine-free, fasting males were given 100 mg orally 24 h before being again with third dose (750 administered every 12 h). Blood samples serially collected after both first last ciprofloxacin. Ciprofloxacin significantly increased half-life (from 5.2 +/- 1.2 to 8.2 2.5 h) area under concentration-time curve 16.3 6.6 25.9 7.8...
The 80-gene molecular subtyping signature (80-GS) reclassifies a proportion of immunohistochemistry (IHC)-defined luminal breast cancers (estrogen receptor-positive [ER+], human epidermal growth factor receptor 2-negative [HER2-]) as Basal-Type. We report the association 80-GS reclassification with neoadjuvant treatment response and 5-year outcome in patients cancer.Neoadjuvant Breast Registry Symphony Trial (NBRST; NCT01479101) is an observational, prospective study that included 1,069...
Abstract Background The Neoadjuvant Breast Symphony Trial (NBRST) demonstrated the 70-gene risk of distant recurrence signature, MammaPrint, and 80-gene molecular subtyping BluePrint, precisely determined preoperative pathological complete response (pCR) in breast cancer patients. We report 5-year follow-up results addition to an exploratory analysis by age menopausal status. Methods observational, prospective NBRST (NCT01479101) included 954 early-stage patients aged 18–90 years who...
As more patients with early-stage breast cancer receive neoadjuvant endocrine therapy (NET), there is a need for reliable biomarkers that can identify HR+ HER2- tumors who are likely to benefit from NET. NBRST (NCT01479101) compared the prognostic value of 70-gene risk classification and 80-gene molecular subtyping signatures conventional pathological methods in response therapy. We evaluated association these clinical 5-year outcome treated NET.1091 scheduled were prospectively enrolled...
Abstract Background. Mammographic wire localization is the primary means of localizing nonpalpable breast lesions for excision. Unfortunately, that procedure associated with patient discomfort and vasovagal responses, scheduling difficulties, problems inaccurate placement or displacement. In addition, wires often enter far from target take an indirect course resulting in high positive margin rates. We reviewed our experience intraoperative ultrasound guided bracketing to determine if...
Abstract Lymphedema is a leading posttreatment complication for many cancer patients. Roughly 1 of 3 women treated breast with surgery, radiation, or taxane based chemotherapy will develop chronic lymphedema. characterized by buildup lymphatic fluid that causes painful and debilitating tightness swelling the extremity decreased range motion increased risk infection. Unfortunately, it rarely diagnosed until has become symptomatic due to fibrotic changes lipid deposition caused protein rich...
The flow reactivity of an internal thoracic artery (ITA) graft and a vein for multiple coronary beds in response to different modes cardiac pacing remains unclear. These experiments were conducted 14 anesthetized dogs with the ITA or grafted circumflex artery, off pump, using brief local occlusion. left main was occluded, rendering entire ventricle totally dependent upon graft. When occluded heart rate 120 beats per min, 93.4 +/- 9.6 ml min ITA, 96.1 10.4 Atrial increase rates 25% 150...
