われわれは,下顎骨切除後の再建において,下顎骨再建プレートと軟組織再建を用いた下顎再建例における合併症の頻度と内容について検討した。対象は,下顎骨切除後に再建プレートと遊離組織皮弁あるいは有茎皮弁により再建を行った22例を対象とした。内訳は,男性14例,女性8例,年齢は35歳から81歳(平均:68.0歳)であった。原疾患は,下顎歯肉扁平上皮癌が最も多く17例,次いで舌下腺腺様嚢胞癌,下顎エナメル上皮腫がそれぞれ2例,下顎軟骨肉腫が1例であった。術前治療を行ったものが13例で内訳は化学療法(C)+放射線治療(RT)が5例,Cのみが4例,RTのみが4例であった。術後治療は7例で,内訳はRTのみが6例,C+RTが1例であった。再建プレートの合併症は6例(27.2%)で,スクリューの緩みが3例,破折,感染,露出がそれぞれ1例ずつであった。スクリューの緩み,破折と感染はいずれも近位端で発現していた。また再建プレートの合併症の発現は,有歯顎で十分に咬合負荷が可能な症例であった。下顎切除後のプレート再建を行う際は,術後合併症を避ける方策をとるべきと考える。
頸動脈の動脈硬化による頸部頸動脈狭窄は,脳梗塞の原因となることが多いとされ,近年重要視されている。自覚症状を認めにくいことから,発見されにくい病変であるが,症例により,パノラマX線画像で異常を指摘できることがある。今回われわれは,2011年4月から2013年3月までに当院歯科口腔外科を受診し,パノラマX線画像および頸部CTを撮影した105例について,頸動脈の石灰化について評価を行った。CT画像では,62例について頸動脈の石灰化を認め,パノラマX線画像で,20例に石灰化物を認めた。また,脳梗塞や,脳梗塞のリスクとされる既往の有無に関わらず認められるほか,年齢に比例して増加することが示された。パノラマX線画像は,歯科・口腔外科を受診する患者で一般的に撮影されており,パノラマX線画像を読影する上では,自覚症状のない頸動脈の硬化性変化がみられる可能性を考慮し読影に当たる必要がある。

2006年10月から2013年3月までに当科で口腔癌と診断し,補助装置を利用してモールド照射を行った8例を検討の対象とした。内訳は,男性6例,女性2例で,年齢は31から94歳(中央値78歳)であった。原発部位は舌が3例,口蓋,頰粘膜がそれぞれ2例,口底が1例であった。モールド照射の治療目的は,外部照射後の再発例に対する根治照射が2例,緩和照射例が2例,術前照射,術後照射,外照射後の追加照射ならびに化学療法後の追加照射がそれぞれ1例であった。照射は8から10回の治療で総線量40から50Gyを施行した。根治照射の2例においては,再発や転移所見はみられず,また緩和照射,術前・術後照射などそれぞれの治療目的に寄与できた。補助装置を利用したモールド照射は,口腔癌治療に対し安全で多様性のある治療法と考える。
