扁平苔癬 (Oral lichen planus,以下OLP) の臨床型分類は,2型に分類するものから,3型,6型の分類あるいはさらに細かく分けるものなどさまざまである。典型例では問題はないと考えられるが,病変のタイプが混在している場合にはその判断が診断医によってばらつく可能性が高い。今後OLPの研究を進めるにあたり,各診断医間で再現性のある臨床型分類を検討した。自治医科大学附属病院歯科口腔外科にてOLPと診断された初診時の病態写真を用いて,日本口腔外科学会専門医5名が各々の臨床型分類を行った。結果としてAxéllの2型分類では診断の一致率が高く,分類数が増えるにつれ一致率は低下した。また,診断は網状型の典型例で一致しやすく,病態が混在する症例では相違が見られた。再現性の高さからは2型分類が適切と考えられ,また今後複合型・多発部位発症型の分類の基準を明確に定義づけすることが必要と考えられる。
