Edwardsiella tarda(以下,E. tarda)による子宮付属器膿瘍で発症し,診断・治療に苦慮した虫垂癌を経験した。起炎菌となったE. tardaは感染性腸炎の原因になるものの,自然治癒することが多いが,免疫不全患者においては腸炎の重症化,軟部組織感染や膿瘍形成の合併などにより致死率も高いため,早期の感染巣除去が必要とされている。 症例は83歳女性で,約1年4か月前に発熱を主訴に受診した。複雑性尿路感染と診断し加療を開始したが,E. tardaによる子宮付属器膿瘍からの敗血症であることがわかった。手術が必要と考え基幹病院へ転送したが,この際は抗菌薬投与のみで治癒した。しかし今回,発熱を主訴に当院救急外来を受診,子宮付属器膿瘍の再燃であることがわかり入院した。抗菌薬投与のみでは改善しなかったため,他の基幹病院へ転送し,手術を行なった。最終的に虫垂癌が原因の子宮付属器膿瘍であることが判明した。
