目 的:センチネルリンパ節生検(SNB)における術中迅速病理診断(FS)省略の影響について検討する.方 法:研究1,2008年1月~2009年12月に原発性乳癌に対してSNBが施行された症例群(N=454)でのFS省略により推定される影響を検討した.さらにACOSOG Z-0011試験対象に相当するcT1-2N0乳癌,乳房温存手術(BCS)+SNB+全乳房照射施行症例(N=308)での影響を検討した.研究2,実際にFS省略を臨床導入した2015年7月~2016年12月のFS省略SNB施行群(N=207)において,安全性を検討した.結 果:研究1,Z-0011試験結果に基づき2個以内のセンチネルリンパ節転移は郭清不要とした場合の腋窩再手術推定施行率は3.2%であった.研究2,腋窩再手術施行率は2.4%,腋窩再発は1例であった.考 察:FS省略はSNBの本質や最終診断には影響がなく,Z-0011試験基準を適用した場合は再手術施行率も充分低く,安全にSNBを施行することが可能であった.

目 的:局所進行乳癌に対する集学的治療の一つとして手術が施行された症例について,後方視的に切除の意義を検討する.方 法:2012年1月~2016年12月に臨床病期Ⅲ(cStage ⅢA-ⅢC)の進行乳癌に対して集学的治療の一つとして手術が施行された136例について臨床病理学的に検討した.結 果:術前化学療法の臨床的治療効果は奏効率86%で,病理学的完全奏功率は15%であった.観察期間中央値7年において局所再発13例(10%),領域再発24例(18%),遠隔再発36例(26%)が認められた.術後5年無再発生存率は65%で,病理学的リンパ節転移によって無再発生存期間に有意差が認められた(p=0.013).また皮膚浸潤例に対して,薬剤治療と広範囲皮膚切除,菱形局所皮弁による集学的治療が有効であった.考 察:局所進行乳癌は原発巣,リンパ節の進行度や生物学的特性に応じた治療が必要であり,T因子の進行に対しては進歩する薬剤治療と広範囲皮膚浸潤に対する菱形局所皮弁などの手術の工夫を組み合わせた集学的治療が有効と思われる.
