A 44-year-old male sustained trauma to his foot leading a 5-cm defect of the first metatarsal bone and infection by Staphylococcus aureus. Osteotomy is most suitable method for treating large defects complicated with osteomyelitis, however few reports have been published on this challenging approach. In case, osteotomy external fixation distraction were performed. Finally, osteomyelitis patient was well controlled, length restored, could carry weight completely, treatment effect satisfactory.
