本研究は,第6頸髄(C6)損傷者における寝返り動作可否の要因を,3次元動作解析装置を用いて分析することを目的とした。C6損傷者11名を対象とし,寝返り動作時の力源である頭部と上肢の手関節の動きを分析した。寝返り可能群と不可能群における頭頂部の変位・速度・加速度が水平・鉛直・奥行き成分すべてにおいて可能群の方が有意に大きかった。手関節の変位・速度は水平・鉛直・奥行きの3成分において共に寝返り可能群の方が不可能群より有意に大きかった。加速度においては水平・鉛直成分でのみ寝返り可能群の方が有意に大きかった。このことから,C6レベルの寝返り動作において,肩関節屈曲方向に振る上肢の振りに続く,大きく・速く・強い頭頸部の動きが必要ではないかと考えられた。

〔目的〕脳性麻痺児の上肢の治療の一つに,選択的筋解離術がある.本研究では,上肢筋解離術後の機能・能力の改善と満足度の関係を明らかにするためにアンケート調査を行った.〔対象と方法〕アンケートは都内某整形外科病院にて肩・肘・前腕部の筋解離術を受けた265名に送付した.アンケートでは,術後の機能・能力の改善の有無(筋緊張・呼吸機能・粗大運動能力・遊び・ADL)と術後満足度を調査した.各機能・能力の改善の有無と満足・不満足における比率の差を検討した.〔結果〕20名から本研究の取り込み基準を満たした返送を得た.粗大運動・ADLが改善した場合,術後に満足している人数が多いことが明らかとなった.〔結語〕粗大運動・ADL能力は,改善が保護者にもわかりやすく上肢選択的筋解離術術後の満足度と関係していることが示唆された.
