症例は 70 歳代男性．右季肋部痛を主訴に来院．腹部単純 X 線にて右上腹部に胆嚢内ガス像を認めた．腹部 CT 検査でも胆嚢気腫像を認め，気腫性胆嚢炎と診断した．同時に肝臓 S7 に 55 mm の腫瘤を認め，精査にて肝細胞癌と診断した．緊急での胆嚢摘出術を行うことで，その後の肝切除術が癒着によって困難になることが予想された．そのため，気腫性胆嚢炎に対して保存的治療を行い，後に胆嚢摘出術及び肝切除術を同時に行うこととした．保存的治療にて全身状態の改善みられたため，入院第 22 病日に胆嚢摘出術及び肝切除術を施行した．気腫性胆嚢炎を契機に見つかった肝細胞癌症例に対して，胆嚢摘出術及び肝切除術を同時に施行した 1 例を経験したので文献的考察を加えて報告する．
