62歳男性. 繰り返す癒着性イレウスの後, 57歳で小腸広範切除を施行し短腸症候群 (short bowel syndrome: SBS) となった. 吸収障害による下痢が続き, 高カロリー輸液を行っていたが, カテーテル感染のため継続困難であった. 下痢による脱水, 感染に加えて高シュウ酸尿症による広範な腎結石が形成され腎不全は進行し, 62歳時に血液透析を導入した. SBS合併のある本例では, 通常透析患者には認められない種々の電解質異常を伴い, またアシドーシスも高度で管理に難渋したが, 標準組成半消化態経腸栄養剤と和漢薬の併用により, 経静脈栄養から離脱しても良好な管理が可能であった.
C 型肝炎ウイルス(HCV)感染透析患者は非感染患者より予後不良であるとされており,HCV 感染の予防・診断・治療は重要な課題である.そこで2020 年に血液透析患者のHCV の保有,検査,治療などの現状に関するアンケートを,千葉県内の透析医療機関に行った.HCV 抗体陽性者は3.7%,HCV-RNA 検査陽性者は1.7%であり,2018 年の全国調査(5.2%,2.5%)より低率であった.肝臓専門医紹介率は41%で同全国調査(22.8%)より高い傾向にあった.抗ウイルス治療はHCV 抗体陽性者の24%に行われており,そのうち71%の患者が直接作用型抗ウイルス薬による治療であった.肝臓癌の治療は過去5 年間にHCV 抗体陽性者の7.5%で行われていた.千葉県内では全国調査結果に比べHCV 感染について高い関心を持ち検査,治療が行われているがまだ改善の余地があると考えられ,より一層の疾患啓蒙,治療,検査の周知が必要と考えられた.
難治性下顎骨骨髄炎に持続的局所抗菌薬灌流（continuous local antibiotic perfusion： CLAP）を施行し，手術を行うことなく感染を制御し得た透析患者の1例を経験した．症例は透析歴20年の66歳男性．齲歯の抜歯18日後に悪寒戦慄と発熱を生じ維持透析施設に入院．抗菌薬投与で改善せず，3日後に当院紹介となった．左下顎から側頭部にかけて膿瘍を認め，緊急ドレナージ術を施行した．感染制御が困難であり，第53病日の造影CT検査で下顎骨の骨融解を認めたため下顎骨切除を検討したが，耐術不能と判断した．その後さらに発熱再燃や白血球数およびプロカルシトニン，CRPの上昇がみられたため，CLAPを下顎骨に対して施行した．CLAPは2週間施行し，副作用なく終了した．CLAP終了後は全身状態も改善し，第97病日に転院した．5か月後のCT検査では膿瘍の再燃はなく，下顎骨も骨形成を認めた．CLAPは透析患者の骨軟部組織感染症に対する重要な治療選択肢となる可能性がある．
