症例は72歳の女性で,心窩部痛と嘔気を主訴に当院を受診した.下腹部に圧痛を伴う可動性腫瘤を触知し,腹部CTで骨盤内に約10cm大の分葉状腫瘍を認め,巨大腹腔内腫瘍の診断で入院した.腹部造影CTとMRIで上腸間膜動脈を軸にした小腸捻転と小腸gastrointestinal stromal tumor(以下,GISTと略記)の疑いと診断し待機的に手術を行った.トライツ靭帯から100cm肛門側空腸に壁外発育性の分葉状腫瘍を認め,腫瘍は茎部で約180度捻転していた.また小腸は腸間膜起始部で時計方向に約270度軸捻転していたが虚血は認めず,腫瘍を含めた空腸部分切除を施行した.病理組織学的検査の結果,high-grade malignancyの空腸GISTと診断した.小腸GISTによる小腸軸捻転の報告は散見されるが,検索した範囲でGIST自体の茎捻転を伴った報告は過去になく,非常にまれな病態であり報告した.

直腸癌患者56例に経直腸超音波検査を行い, 56例中転移陽性28例中の15例, 転移陰性28例中の4例に旁直腸リンパ節が描出できた.56例の手術標本から得た791個の旁直腸リンパ節の大きさとリンパ節内に占める転移の範囲と, 描出の有無を検討してみると, 転移陽性のリンパ節の20%転移陰性のリンパ節の0.7%が描出され (P<0.01), 転移陽性のものでは転移組織がリンパ節の半分以上を占める4mm以上のリンパ節が描出され, また転移陰性でも大型のリンパ節には描出されたものがあった.しかし画像上からは転移の有無を区別できなかった.寒天で作成したリンパ節の模型を用いると球状のものは偏平なものに比べ描出されやすく, また転移組織がリンパ節の半分以上を占めるリンパ節の多くは球状で, 転移が小範囲のものや陰性のものは偏平なものが多かった.球状に腫大したリンパ節が描出されやすいと思われた.
