The stomach is a sensitive digestive organ that susceptible and exposed to exogenous pathogens from the diet. In response such pathogens, induces oxidative stress, which might be related development of gastric organic disorders as gastritis, ulcers, cancer, well functional dyspepsia. particular, bacterium Helicobacter pylori plays major role in eliciting confronting stress stomach. present paper summarizes pathogenesis during various diseases.
Abstract Background Rikkunshito, a standardized J apanese herbal medicine, is thought to accelerate gastric emptying and relieve dyspepsia, although no large‐scale, randomized, placebo‐controlled trials of rikkunshito have been conducted. This study aimed determine the efficacy safety for treating functional dyspepsia ( FD ). Methods patients received 2.5 g or placebo three times day 8 weeks in this multicenter, placebo‐controlled, parallel‐group trial. The primary end point was proportion...
Abstract Background and Aims: Although clarithromycin (CLR) is one of the most commonly recommended component drugs Helicobacter pylori eradication regimens, prevalence CLR‐resistant H. has been increasing. It well known that CLR resistance associated with point mutations in 23S rRNA, but an active multidrug efflux mechanism may also play a role its drug resistance. At least four gene clusters have identified as pump systems present study was designed to investigate their clinical isolates ....
Background: Helicobacter pylori eradication rates achieved with a first-line regimen of clarithromycin (CLR) combined amoxicillin (AMX) and proton pump inhibitor have recently fallen to ≤80% because the increasing incidence CLR resistance in Japan. This randomized multicenter trial aimed compare success 2 triple therapy regimens: rabeprazole, amoxicillin, (RAC) versus metronidazole (RAM). Methods: A total 124 consecutive patients infected H. were into one two 7-day therapeutic RAC (n=60) or...
Duodenal endoscopic submucosal dissection (ESD) is difficult due to insufficient mucosal elevation, because of the coarse layer [1]. We report on "water pressure method" for duodenal ESD.
Abstract In the previous year, some extragastric diseases, possibly linked to Helicobacter pylori infection, have been largely investigated. There are, in fact, several studies concerning cardiovascular lung hematologic eye and skin hepatobiliary diabetes mellitus, neurological disorders. Among them, relationship between bacterial CagA positivity coronary heart disease is reportedly emphasized. Concerning normal tension glaucoma, new interesting data are playing favor of association with H....
Abstract Background and study aims Mucosal suturing enables reliable optimal defect closure after endoscopic treatments. We developed investigated the feasibility of hand-suturing (EHS) gastric submucosal dissection (ESD) in porcine vivo models human clinical cases. Patients methods EHS involving continuous mucosal layers using a through-the-scope needle-holder absorbable barbed suture, was performed ESD six live pigs eight consecutive patients. Success rates, adverse events, suture...
Currently, a standard third-line therapy for Helicobacter pylori (H. pylori) eradication remains to be established. Quinolones show good oral absorption, no major side effects, and marked activity against H. pylori. Several authors have studied quinolone-based reported encouraging results, with the cure rates ranging from 60% 84%. Resistance quinolones is easily acquired, resistance rate relatively high in countries consumption of these drugs. We recently significant difference obtained...
ABSTRACT Sitafloxacin-based triple therapy achieved 83.6% (per-protocol) and 78.2% (intention-to-treat) success in eradicating Helicobacter pylori among 78 Japanese patients after clarithromycin-based first-line metronidazole-based second-line therapies failed. Eradication succeeded 32 out of 43 patients, even with gyrA mutation-positive (per protocol). The position the mutation (N87 or D91) was determined to be a better marker than MIC levels for predicting outcomes sitafloxacin-based treatment.
Since the publication of Rome III criteria for functional dyspepsia (FD), evidence about efficacy half-dose proton pump inhibitors symptoms have been limited. To examine lansoprazole (FD) diagnosed with by multicentre, double-blind, randomized, placebo-controlled study in Japan.A total 54 FD participants were randomized to 15 mg once daily or placebo a 4-week double-blind treatment period. The primary endpoint was an overall dyspeptic symptom relief rate evaluated 5-point Likert scale scores....
